The 72-page Food & Activity Log is a 4-week replaceable daily food log for the
HealthChequesTM: Self-Monitoring System (Item #400)

Record all the foods and beverages you eat and drink throughout the day. Afterwards, either track the nutrients you've chosen to monitor or place a checkmark in the food group box provided on the left side of the page.  Compare how your eating habits stack up against your desired eating pattern plan (goal) or against MyPlate.gov. 

You can record the calories and fat grams as found in the upper part, Food & Activity Counts in the columns provided. Use the additional column to monitor carbs, carb choices, sodium, fiber, protein or saturated fat. At the bottom of the page, record your goals for eating pattern and physical activity (e.g., “My goal is to eat 4 servings of vegetables today.” “My goal is to walk 10,000 steps today.”). There’s a place to record either the number of steps on a pedometer or the number of minutes spent doing a physical activity (walking, swimming, bicycling, etc.).

Sample eating pattern plans are provided ranging from 1200 to 3000 calories a day.
